Alliston Christian Reformed Church New Pastoral Care Structure:

Adult Discipling Ministries:
Pastoral Care:
  • Each small group leader is primarily responsible for the pastoral care of those people in their groups
  • For transition purposes, the adult discipleship team is made up of people elected as pastoral elders.  Each pastoral elder has a list of people not in a small group.
  • The pastor has a role in providing pastoral care, but is not the primary source of care.  Their level of involvement is determined by their spiritual gifts.
  • As our ministry matures and develops, pastoral care will become more and more group oriented; whether an official small group, or house group, or any other short term or long term ministry group.

CrossPoint Christian Reformed Church Pastoral Care Visitation
CrossPoint CRC has pastoral elders who are trained as visitors.  They keep track of their visits and report back to the Pastor of Pastoral Care and meet together for mutual encouragement and training.  This is an example of the form they use to keep track of their visits:
